Insurance and Government Payers

The practice is accepting new patients. Please call.

For new patients, insurance contracts accepted are: BlueCross/BlueShield PPO (NOT Indemnity/CostWise/Classic Blue) and CIGNA.

Please note: If I do not currently accept your insurance, full payment is required at the time of service. In this situation, I will provide a bill that can be sent to your insurance company for reimbursement. Please consider this if you are planning to become a new patient and I do not have a contract completed with your insurer.

Accepting insurance payments, filing claims, collecting balances, and complying with a multitude of insurance company requirements is very expensive for a small practice. I cannot ensure that I will continue to be able to participate with these insurances.

Visit Payments
During your visit, please be prepared to pay the amount you are responsible for. This includes co-payments, non-covered services, and self payment. I accept major credit and debit cards, cash, and checks.

Additional charges may arise during your visit, including procedures and tests.

Charges that are subsequently not covered by your insurance will be billed to you. Prompt payment is important for keeping the practice running.

Discounts for Visits
Several discounts are available:

  • All insurance and goverment payers accepted include discounts from the usual charges.
  • Full payment at the time of service is awarded a discount due to the high costs of collecting charges later. This applies to anyone, but you should review how accepting the discount affects your insurance plan. In addition, your insurance carrier may prohibit me from accepting the full charge at the time of service.
  • Financial hardship discounts are available. If you have a need for this, please bring proof of income to your first visit as this must be pre-arranged. This discount is available to all patients, but the number of discounts available is necessarily limited. Please ask whether any are available prior to making an appointment.
  • Annual Fee
    Insurance programs usually cover only face-to-face visits. In order to continue the higher level of services outside of visits, it has become necessary to add an annual fee of $75. This is typically not covered by insurance programs. At your first visit, a $75 ($40 January to June) registration fee is due, and the annual fee is due each July 1. There is a sponsorship program to help those who have financial difficulty with this fee.
