A most helpful tool for reaching our goal is How's Your Health. This website walks you through your health status. It offers great ideas on how you can help yourself. I strongly encourage you to choose the option to send the information it offers to me so that we can work on it together!

How's Your Health does not store ANY information about you personally. You can print information for yourself and to bring to your visit, but you decide where that information goes.
Contribute to Quality
In addition to providing us valuable information to use to improve your health, your entries into How's Your Health help me improve the care everyone receives at One Family Doctor. When you use the patient link here or enter my access code (IMP412), your information is combined with other patients' so I can get a better idea of how I am doing overall. If a number of patients have similar problems, I can change the practice to help everyone. This is a powerful way to improve health care and is supported by a lot of studies and experience.
When?
You can take the How's Your Health survey at any time. After your initial entry, I encourage you to repeat the survey at least when you have a preventive physical, and to use the e-mail or print option so we can review it at your visit.
